Job summary

Moments Hospice is seeking an RN Case Manager to lead the clinical care for patients and families, managing symptoms, coordinating care, and guiding families through the end-of-life journey.

You will work in a supportive setting with reasonable caseloads, access to leadership, and a field-based schedule. The role includes a fleet car program and comprehensive benefits.

Qualifications

• Active RN license in the state of work.
• Proficiency with EMR systems.
• Strong organizational and communication skills.
• Ability to work independently.
• Compassionate patient- and family-centered approach.
• Valid driver's license and reliable vehicle.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
