Job summary

Comfort Hospice in Nevada seeks a dedicated Registered Nurse to act as a trusted Case Manager, delivering in-home hospice care and guiding families through care decisions with compassion. The role involves skilled nursing, medication administration, education for patients and families, and meticulous documentation while collaborating with an interdisciplinary team.

Travel throughout the Pahrump area is required and a Nevada RN license is essential.

Qualifications

• Current RN license in the State of Nevada
• Minimum of 2 years of nursing experience, hospice or home health experience preferred
• Homecare/Homebase experience preferred, not required

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs
• Develop and implement individualized care plans, revising as needed
• Provide skilled nursing care, administer medications, and manage symptoms
• Educate patients and families on care techniques, medications, and comfort measures
• Maintain accurate clinical documentation and communicate changes to physicians
• Participate in interdisciplinary team meetings and quality improvement initiatives
• Provide emotional support and counseling to patients and families
• Supervise hospice aides and delegate responsibilities appropriately
• Collaborate with an interdisciplinary team that shares your commitment to excellence
• Participate in holiday on-call rotation as needed

*This is a full-time position requiring travel throughout the Pahrump area!

Requirements

• Current RN license in the State of Nevada
• Minimum of 2 years of nursing experience, hospice or home health experience preferred
• Homecare Homebase (HCHB) experience preferred, not required
• Valid driver's license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
• Ability to work independently and a passion for making a real difference
