Nurses At Home in Hartford, CT is seeking a full-time RN Case Manager to oversee a home health caseload, coordinate interdisciplinary care, and maintain clinical excellence and Medicare compliance. The role offers autonomy with clinical leadership support.

Ideal candidates are organized, patient-centered, and capable of independent work, with an active CT RN license, at least 1 year in home health, CPR, and a valid driver's license. Competitive salary and benefits.

Formación

• Active Connecticut RN license.
• Minimum 1 year home health nursing experience.
• Strong clinical judgment and assessment skills.
• Excellent time management and communication skills.
• Valid driver’s license and reliable transportation.
• CPR certification.

Responsabilidades

• Perform Start of Care (SOC), Recertifications, ROCs, and routine skilled nursing visits.
• Develop and implement individualized care plans.
• Provide patient and caregiver education.
• Coordinate care with physicians and interdisciplinary team members.
• Ensure compliance with Medicare regulations and agency policies.
• Complete accurate and timely documentation in EMR.
• Participate in case conferences and team meetings.
