Job summary

RVNAhealth is seeking a compassionate Registered Nurse Case Manager to provide exceptional home health care across our Central Territory in CT, including Weston, Wilton, and Redding. You will manage your own caseload, coordinate interprofessional care, and support patients to recover and remain safely at home.

Qualifications

• Graduate of an accredited School of Nursing with active RN licensure.
• Current Registered Nurse license in Connecticut or Compact RN license.
• Minimum of 1 year nursing experience.

Responsibilities

• Manage a caseload of home health patients with skilled nursing and case management.
• Conduct Start of Care admissions and ongoing assessments.
• Develop and update individualized plans of care with physicians and team.
• Coordinate nursing, home health aide, therapy, and support services.
• Monitor progress, adjust care plans, and communicate with physicians.
• Educate patients and caregivers on diagnoses, medications, and self-management.
• Complete timely clinical documentation per guidelines.
• Participate in discharge planning and care transitions.
