Yale New Haven Health is seeking a Registered Nurse Case Manager to coordinate home care services, develop patient plans of care, and ensure safe delivery of care across disciplines in Connecticut.

Requires RN license in CT, CPR certification, and ability to travel as needed. Two years acute care experience; home health preferred. Strong communication and organizational skills are essential.

Formación

• RN with Nursing Diploma or ADN; BSN preferred.
• Minimum two years in acute care; home health experience desirable.

Responsabilidades

• Assesses/reassesses patients and develops patient-specific plans of care.
• Collaborates with providers and disciplines; communicates changes promptly.
• Ensures ongoing authorizations and facilitates documentation.
• Educates patients and caregivers; begins discharge planning on admission.
• Completes all documentation within 24 hours with supporting notes.
• Oversees LPNs and Home Health Aides; coordinates scheduling.
• Participates in after-hours, weekend, and holiday coverage as needed.
• Maintains OASIS documentation and follows policy procedures.
