Job summary

Nurses At Home is actively seeking a Registered Nurse Case Manager to join our Hartford area team. This full-time role involves managing a caseload of home health patients, coordinating interdisciplinary care, and maintaining high clinical standards to ensure exceptional patient experiences.

The ideal candidate will hold an active Connecticut RN license, have at least 1 year of nursing experience (home health preferred), and demonstrate strong clinical judgment, organization, and communication

Qualifications

• Active Connecticut RN license.
• Minimum 1 year nursing experience (home health preferred).
• Strong clinical judgment and assessment skills.
• Excellent time management and communication skills.
• CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform Start of Care (SOC), Recertifications, ROCs, and routine skilled nursing visits.
• Develop and implement individualized care plans.
• Provide patient and caregiver education.
• Coordinate care with physicians and interdisciplinary team members.
• Ensure compliance with Medicare regulations and agency policies.
• Complete accurate and timely documentation in EMR.
• Participate in case conferences and team meetings.

We are growing in the Hartford region and are seeking a Full-Time RN Case Manager to join our team.

Position Overview

The RN Case Manager is responsible for managing a caseload of home health patients, coordinating interdisciplinary care, and ensuring clinical excellence, compliance, and outstanding patient experience.

This position offers autonomy in the field with strong clinical support from leadership.

Schedule

• Monday through Friday

• Weekend on-call rotation (shared among clinical team)

Responsibilities
