Job summary

Elara Caring is seeking a Registered Nurse Case Manager to deliver home health care across Avon, Lorain, Elyria and surrounding Lorain County.

You will conduct initial and ongoing assessments, develop and implement care plans, coordinate with physicians, and educate patients and families for safe, effective home care. Schedule is Monday–Friday 8:00 a.m.–5:00 p.m. with a weekend rotation; no on-call.

Qualifications

• Current RN license valid in Ohio.
• Graduation from an accredited nursing program (certificate/diploma/associate/bachelor).
• At least 1 year of nursing experience in a clinical setting.
• Home health experience preferred, not required.

Responsibilities

• Complete initial and ongoing assessments to identify patient needs.
• Provide nursing care per individual care plans and physician orders.
• Monitor patient conditions, report changes, and take appropriate action.
• Coordinate with physicians and care team to support continuous care.
• Educate patients and families on how to manage care and safety concerns.
• Maintain accurate clinical documentation and protect patient privacy.
• Use clinical judgment and timely communication to support outcomes.
