Elara Caring in Mentor, OH is seeking a Registered Nurse Case Manager to deliver high-quality home health services across Lorain County. You will assess patients, develop plans of care, coordinate with the care team, and educate families to ensure safety and positive outcomes.

The role requires an active Ohio RN license, 1+ year of clinical nursing experience, and the ability to travel between patient homes. A valid driver’s license and reliable transportation are essential.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
