Job summary

Elara Caring is hiring an RN Case Manager to deliver home health care across Trumbull and Mahoning counties in Ohio. You will organize patient visits, provide individualized care, and help patients remain safe in their homes.

The role requires an active RN license, 1+ year of clinical experience, and the ability to travel up to 50% with lifting up to 50 pounds. A Bachelor's degree is preferred and HomeCare HomeBase experience is a plus.

Qualifications

• Current, unrestricted state RN license.
• 1+ year of clinical care experience as a nurse.
• Willingness to travel within branch/office coverage area (50%).
• Ability to lift up to 50 pounds and assist patient transfers.
• Valid driver's license or reliable transportation.

Responsibilities

• Assess patients and develop individualized care plans.
• Provide skilled nursing care in patients' homes.
• Educate patients and caregivers on medications and treatments.
• Coordinate care with physicians and interdisciplinary team.
• Complete timely, accurate clinical documentation.
