Job summary

Elara Caring in Ohio seeks a Registered Nurse Case Manager to deliver patient-centered home health care. You’ll build relationships, assess needs, and guide recovery for patients in Avon, Lorain, Elyria, and surrounding communities.

This role offers weekday schedules with a weekend rotation, no on-call requirement, a supportive preceptor program, and comprehensive benefits to help you provide Right Care at the Right Time.

Qualifications

• Current RN license valid in Ohio.
• Graduation from an accredited nursing program.
• At least 1 year of nursing experience in a clinical care setting.
• Home health experience preferred; not required.

Responsibilities

• Complete initial and ongoing assessments to identify patient needs.
• Provide nursing care according to individualized care plans and orders.
• Monitor patient conditions, report changes, and take appropriate action.
• Coordinate with physicians and care team to support continuous care.
• Teach patients and families how to manage care and recognize safety concerns.
• Maintain accurate clinical documentation and protect privacy.
