Job summary

Elara Caring is seeking a Registered Nurse Case Manager to support patients in Avon, Lorain, Elyria, and surrounding Lorain County communities. You will provide individualized home health care, build patient relationships, and guide recovery with expert nursing care in familiar surroundings.

The role offers training with a preceptor, a Monday–Friday schedule with weekend rotation, and no on-call requirement. Orientation is in Mentor, OH, with patient coverage in Lorain County.

Qualifications

• Current RN license valid in Ohio.
• Graduation from an accredited nursing program (certificate, diploma, associate, or bachelor’s).
• At least 1 year of nursing experience in a clinical care setting.

Responsibilities

• Complete initial and ongoing assessments to identify patient needs.
• Provide nursing care according to individualized care plans and physician orders.
• Monitor patient conditions, report changes, and take appropriate action.
• Coordinate with physicians and the care team to support safe, continuous care.
• Teach patients and families how to manage care and recognize safety concerns.
• Maintain accurate clinical documentation and protect patient and staff privacy.
• Use clinical judgment and timely communication to support quality patient outcomes.

Your nursing skills can help a patient move through recovery with greater confidence. As an RN Case Manager with Elara Caring , you’ll provide individualized care, get to know your patients, and help them manage their health in familiar surroundings.

We’re seeking a Registered Nurse to serve Avon, Lorain, Elyria, and surrounding Lorain County communities . Whether you bring home health experience or are ready to explore a new setting, you’ll receive training with a preceptor to support your transition.

Know What to Expect

• Schedule: Monday through Friday, 8:00 a.m. to 5:00 p.m., with a weekend rotation.
• No on-call requirement.
• Orientation location: Mentor, Ohio. Patient coverage is in Lorain, County.

What You’ll Need

• Current RN license valid in Ohio.
• Graduation from an accredited certificate, diploma, associate, or bachelor’s nursing program.
• At least 1 year of nursing experience in a clinical care setting .
• Home health and Homecare Homebase experience preferred. Home health experience is not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
