Job summary

Elara Caring is seeking a Registered Nurse Case Manager to join our home health team serving Lorain County, Ohio. In this role you will assess patient needs, complete initial and ongoing assessments, implement and monitor the plan of care, and coordinate care with the medical team.

You must hold a current RN license and graduate from an accredited nursing program; 1+ year clinical experience; home health preferred. Valid driver’s license or reliable transportation required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate from an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred but not required.
• Ability to frequently lift and move up to 50 pounds; valid driver's license or reliable transportation.

Responsibilities

• Assess patients to determine care needs and complete initial and ongoing assessments.
• Provide nursing care according to the plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on care and safety.
• Protect patient and staff privacy under HIPAA and maintain safety.
• Take action when patient conditions change to ensure quality care.
