Responsibilities

• Review and maintain client POC compliance and timely documentation.
• Coordinate care with Authorization Specialists for admissions and recertifications.
• Provide education to clients and families about care plans and services.
• Maintain open communication with DON, ADONs, and care team.
• Attend case management meetings and participate in staff education.
