Job summary

Unifi Healthcare is seeking an RN Case Manager to ensure quality patient care in home settings. You will develop, implement, and evaluate patient care plans, coordinating with physicians and families to achieve optimal outcomes.

You will review charts, manage caseloads, and provide education to patients about benefits and services. Strong communication, organization, and leadership are essential for success in this role within our growing home health team.

Qualifications

• Current RN licensure is required.
• Minimum 3 years progressively responsible clinical experience.
• Excellent writing, organizational, and time management skills.
• Prior management or leadership experience is preferred.
• Ability to work independently, multi-task, prioritize and function in a team.

Responsibilities

• Performs documentation/chart reviews to ensure accuracy and adherence to POC.
• Maintains information in QuickBase for all clients.
• Manages care implementation with emphasis on quality patient care and family satisfaction.
• Coordinates care authorization for admissions and re-certifications.
• Provides education to clients and families about benefits and services.
• Maintains ongoing responsibility for assigned caseload.
• Attends MD appointments and coordinates coverage when needed.
• Builds rapport with patient families and supports staff through coaching.
