Job summary

Unifi Healthcare in the United States is seeking an experienced RN Case Manager to develop, implement, and evaluate patient care plans in home settings. You will coordinate clinical activities, act as a liaison with facilities and families, and ensure compliance with agency policies while delivering compassionate, patient-centered care.

Responsibilities include chart reviews, care coordination, education for families, and collaboration with physicians, DON, and staff.

Qualifications

• Current RN licensure.
• Minimum 3 years progressively responsible clinical experience.
• Excellent writing, organizational, and time management skills.
• Ability to work independently, multi-task, prioritize, and function in a team environment.
• Must possess a professional, courteous, compassionate and customer-oriented demeanor.

Responsibilities

• Performs documentation/chart review to ensure accuracy and that the client’s POC is being followed.
• Maintains accurate information in QuickBase for all assigned clients.
• Coordinates care authorization with Authorization Specialist(s) to establish the foundation for the development of the client’s treatment plan of care.
• Provides implementation, evaluation, and oversight of individual patient P.O.C. that corresponds with the physician’s orders.
• Provides education to client regarding benefits, utilizing appropriate team members within the company structure and/or outside agencies to accomplish desired objective.
• Maintains an ongoing responsibility for assigned caseload.
• Attends all pertinent MD appointments with the client and coordinates coverage as needed.
• Builds rapport with patient families in implementing and managing care being rendered.
• Assists staffing department with scheduling work load for maximum efficiency and deadlines.
• Maintains open communication with DON, ADON(s), other Case Managers, RN Authorization Specialist(s), non-licensed staff, support staff, and ancillary services.
