Job summary

UNIFI Healthcare is seeking an RN Case Manager to ensure quality patient care in home settings. You will develop, implement, and evaluate individualized care plans, coordinating with physicians, families, and the care team. The role emphasizes patient-centered care, documentation accuracy, and seamless communication across departments.

The RN Case Manager reports to nursing leadership and collaborates with a multidisciplinary team to maintain high standards of care and program compliance.

Qualifications

• Current RN licensure.
• Minimum 3 years progressively responsible clinical experience.
• Excellent writing, organizational, and time management skills.
• Prior management or leadership experience.
• Ability to work independently, multi-task, prioritize, and function in a team environment.
• Must possess a professional, courteous, compassionate and customer-oriented demeanor.
• Must be discrete, trustworthy, and possess a high level of integrity.
• Excellent computer and internet skills.
• Must be able to do bending and lifting required for effective patient related care.
• Must be able to safely lift 40-50 lbs.
• Must dress in a professional manner when visiting clients (scrubs and nametag preferred).

Responsibilities

• Performs documentation/chart review to ensure accuracy and that the client’s POC is being followed.
• Maintains accurate information in QuickBase for all assigned clients.
• Manages care implementation with attention to quality patient care and patient family satisfaction.
• Coordinates care authorization with Authorization Specialist(s) to establish the foundation for the development of the client’s treatment plan of care.
• Provides implementation, evaluation, and oversight of individual patient P.O.C. that corresponds with the physician’s orders through assessment of patient’s needs.
