Job summary

Unifi Healthcare is seeking an RN Case Manager to oversee home health plans, coordinate care with physicians, and ensure quality patient care. You will manage documentation, billing inputs, and communication with families to support treatment goals.

The role requires RN licensure, 3+ years clinical experience, and leadership abilities to guide staff and ensure compliance with agency policies. A field-based, on-the-road schedule is expected.

Qualifications

• Current RN licensure.
• Minimum 3 years progressively responsible clinical experience.
• Excellent writing, organizational, and time management skills.
• Prior management or leadership experience.
• Ability to work independently, multitask, prioritize and function in a team.

Responsibilities

• Reviews documentation and ensures POC is followed and billed timely.
• Maintains accurate client data in QuickBase and updates promptly.
• Coordinates care with physicians, authorization, and treatment plans.
• Provides education to clients and coordinates with DME and services as needed.
• Supports scheduling, meetings, and compliance with agency policies.
