Unifi Healthcare is hiring an RN Case Manager to ensure quality patient care in home settings. You will develop and evaluate individual care plans, serve as liaison between the agency and patient families, and supervise clinical activities within your service area.

Reporting to the Director of Nursing, you will collaborate with an interdisciplinary team to promote compassionate, accessible home healthcare across the U.S.

Qualifications

• Current RN licensure.
• Minimum 3 years progressively responsible clinical experience.
• Excellent writing, organizational, and time management skills.
• Prior management or leadership experience.
• Ability to work independently, multi-task, prioritize, and function in a team environment.
• Professional, courteous, compassionate and customer-oriented demeanor.
• Discrete, trustworthy, and possess a high level of integrity.
• Excellent computer and internet skills.
• Able to bend and lift as required for patient care.
• Able to safely lift 40–50 lbs.
• Professional attire when visiting clients (scrubs and nametag preferred).

Responsabilités

• Documentation/chart review to ensure POC accuracy and weekly billing/ payroll submissions.
• Maintain accurate information in QuickBase for all clients.
• Manage care implementation with focus on quality patient care and client family satisfaction.
• Coordinate care authorization to establish treatment plans for admissions and re-certifications.
• Provide implementation, evaluation, and oversight of patient care plans per physician orders.
• Educate clients about benefits using internal and external resources.
• Maintain caseload responsibilities and deadlines.
• Attend MD appointments and coordinate coverage when needed.
• Build rapport with patient families and support team collaboration.
• Assist staffing with scheduling to maximize efficiency and meet deadlines.
• Communicate important updates to DON, ADONs, operations, and billing.
• Provide clinical guidance and coaching to staff and families.
• Participate in Case Management meetings and ongoing education.
