Job summary

UNIFI Healthcare is seeking a skilled RN Case Manager to support home-based care in the Charlotte area. You will review documentation, ensure adherence to the POC, and submit weekly billing and payroll information by deadlines.

You will coordinate care authorizations, develop and oversee patient plans, educate clients about benefits, and maintain an active caseload while engaging with families and the care team to deliver compassionate, compliant services.

Qualifications

• Current RN licensure in good standing.
• Minimum 3 years progressively responsible clinical experience.
• Excellent writing, organizational, and time management skills.
• Prior management or leadership experience.
• Ability to work independently, multi-task, prioritize, and function in a team.
• Professional, courteous, compassionate, customer-oriented demeanor.
• Excellent computer and internet skills.
• Able to lift 40-50 lbs and perform bending as part of patient care.
• Professional appearance when visiting clients.

Responsibilities

• Documentation/chart review to ensure accuracy and client POC adherence.
• Submit clinical documentation within deadlines.
• Maintain accurate QuickBase information for assigned clients.
• Coordinate care authorization and develop the treatment plan.
• Provide implementation, evaluation, and oversight of patient POC.
• Educate clients about benefits with team involvement.
• Maintain ongoing caseload responsibilities and attendance at MD appointments.
• Build rapport with patient families and support team communications.
• Assist staffing with scheduling and ensure deadlines are met.
• Comply with agency policies and state/federal regulations.
