Westminster Canterbury Richmond is seeking a Registered Nurse Case Manager to work independently with groups of clients, assessing skilled needs and implementing care plans. You will coordinate with physicians, families, and a multidisciplinary team to ensure high-quality home health and hospice services in the Richmond area.

The role requires an RN license in Virginia, CPR certification, and at least two years of nursing experience, preferably in hospice or home health.

Qualifikationen

• Current and unrestricted RN license in Virginia.
• Two years nursing experience, preferably hospice/home health.
• Current CPR certification for healthcare provider.
• Basic computer skills.
• Valid Virginia driver’s license with clean driving record.

Aufgaben

• Complete initial and ongoing client assessments and revise care plans.
• Provide skilled nursing care including IVs, catheters, and wounds.
• Counsel clients and families on nursing needs.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes.
• Coordinate services across the care team.
• Refer to other services as needed.
• Inform physician of changes in patient needs.
• Evaluate outcomes and perform OASIS assessments.
• Supervise LPN/LVNs and Home Health & Hospice Aides.
• Assign aides to patients and participate in in-service programs.
• Plan discharge from service.
