Westminster Canterbury Richmond is seeking a Registered Nurse Case Manager to work independently with groups of clients, assessing skilled needs and implementing care plans. You will coordinate with physicians, families, and a multidisciplinary team to ensure high-quality home health and hospice services in the Richmond area.
The role requires an RN license in Virginia, CPR certification, and at least two years of nursing experience, preferably in hospice or home health.
As the Registered Nurse Case Manager, you will work independently with groups of clients to provide initial and on-going assessment of the client’s skilled needs. Implement individual plans of care and make referrals to others disciplines and/or revise the plan as necessary. Provide ongoing consultation and education for patient/family member regarding all aspects of care. Communicate and collaborate with family /caregivers and other professionals involved in the care of the client. Supervise Licensed Practical Nurses and Home Health & Hospice Aides.
Full-Time, 8:30 am to 4:30 pm (Monday through Friday) (Leadership; On-call Rotation)
The hiring range for this position is$33.15 to $50.03.Actual compensation depends on experience, skills, and internal equity.
