Job summary

LeadingAge Virginia is seeking an RN, Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to maintain the participant in their home environment.

The role requires an active RN license in the working state and at least 1 year of nursing experience; hospice or hospital settings are preferred, with EMR/EHR system experience and a valid driver’s license. Travel to home visits is expected.

Qualifications

• Active RN license in the state where working.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with an EMR/EHR system.
• Mobile Driver—valid license and automobile insurance as per policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT, with psychosocial, spiritual, cultural, and physical components.
• Develops and updates the Plan of Care reflecting evolving patient needs.
• Educates patients, families, and other health professionals about disease, prevention, and palliative care.
• Determines visit frequency based on risk/needs.
• conducts home visits to assess safety, medication, nutrition, and DME compliance; reports changes to IDT.
• Participates in daily IDT meetings; communicates clearly with physicians, staff, patients, and families.
• During emergencies, may report to a designated location to ensure continuity of services.
• Performs other duties as assigned.
