Community Hospice is seeking an RN Case Manager to plan and manage nursing care for an assigned caseload in home or long-term care settings. You will educate patients and families, coordinate care across sites, and collaborate with the interdisciplinary team to achieve patient-centered goals.

This role requires flexibility across care settings and strong communication, clinical judgment, and documentation skills.
