Job summary

Mission Healthcare is seeking an RN for a Per Diem/PRN role in Maricopa, AZ. You will perform initial patient assessments, develop and adjust care plans, and provide ongoing nursing care and education to patients and families.

The position requires a current RN license, at least 1 year of acute care experience, and BLS CPR certification. Valid driver's license and home health experience are preferred; benefits are provided as listed.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• BLS certification with hands-on CPR.

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Regularly re-evaluates patients' needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and involves patient and family.
• Initiates preventive and rehabilitative nursing procedures and administers medications as prescribed.
• Counsels patient and family on the plan of care and related needs.
• Provides health care instructions per assessment and plan.
• Identifies discharge planning needs and prepares for discharge.
