Job summary

Mission Healthcare in Maricopa, AZ seeks a compassionate Registered Nurse for per diem/PRN home health assignments. You will perform initial assessments, establish and update care plans, and deliver medications and treatments as prescribed by physicians.

You should hold an active RN license with BLS certification and have at least 1 year of acute care experience; home health experience is preferred. A valid driver's license and auto insurance are required. Relocation assistance may be available.

Qualifications

Current and valid state RN license.
Minimum of 1 year of recent acute care experience in an institutional setting.
Basic Life Support (BLS), with hands-on CPR certification.
Valid driver's license and auto insurance.
Associate's or Bachelor's degree in Nursing, and 1 year of home health experience preferred.

Responsibilities

Performs initial patient and family assessment to determine home care needs.
Initiates plan of care and revises as patient status changes.
Regularly re-evaluates patient needs.
Uses health data to determine nursing diagnoses.
Develops a care plan with goals based on nursing diagnosis and family involvement.
Delivers prescribed medications and treatments per physician.
