Job summary
Home Sweet Home Health is seeking a Registered Nurse (RN) to join our home health team in Arizona. The role covers Maricopa County and surrounding East Valley communities, with full-time or per diem schedules and travel to patients’ homes.
Ideal candidates hold an Arizona RN license (BSN preferred) and have at least 1 year of RN experience, preferably in home health or geriatric care. We offer autonomous mobile nursing with a focus on compassionate, high-quality in-home care.
Qualifications
- Current Arizona RN license in good standing.
- Graduate of an accredited nursing program (BSN preferred).
- Minimum 1 year RN experience; home health experience preferred.
Responsibilities
- Provide skilled nursing services in patients' homes across Maricopa County.
- Deliver wound care, medication management, and treatments.
- Develop, implement, and update individualized home health plans.
- Educate patients and caregivers on disease management and safety.
- Collaborate with physicians and interdisciplinary teams.
- Maintain accurate clinical documentation per standards.
- Monitor progress and communicate changes to the care team.
- Support improved outcomes and patient independence.
