Your Role

As a PRN RN Case Manager, you’ll assess patient needs, plan and coordinate care, and ensure delivery of high‑quality home health services. You will work 10‑30 hours a week and play a key role in a team dedicated to improving patients’ lives.

Compensation & Benefits

• Location: Anthem, AZ
• Flexibility: PRN position, 10‑30 hours per week
• Compensation: $45.00–$120.00 per hour
• Potential annual earnings:
  • 10 hours/week: ~$42,900 per year
  • 30 hours/week: $128,700 per year

Qualifications

• Graduate from an approved school of professional nursing (Baccalaureate degree preferred)
• Current RN license required by the state of Arizona
• Ability to supervise and direct members of the client care team, with demonstrated organizational and time‑management skills
• Strong written and verbal communication skills, good interpersonal skills, and tactful client interaction
• Licensed driver with an insured automobile and reliable transportation (good driving record)
• Current CPR certification
