Job summary

Mission Healthcare seeks a skilled Registered Nurse to provide in-home care across approved territories in Arizona, including New River, Anthem, North Peoria, and North Glendale. You will assess patients, implement and adjust plans of care, and collaborate with families and physicians to promote comfort and independence.

Qualifications

• Active RN license and ability to practice in the state of Arizona.
• Minimum 1 year of recent acute care experience in an institutional setting.
• Current BLS certification with CPR training.
• Valid driver’s license and auto insurance.
• Diploma or degree in Nursing with home health experience preferred.

Responsibilities

• Completes initial patient and family assessment to determine home care needs.
• Initiates and revises the plan of care as patient status changes.
• Regularly reevaluates patient needs and progress.
• Uses assessment data to determine nursing diagnoses.
• Develops and implements a care plan with goals based on diagnoses.
• Administers medications and treatments as prescribed.
• Counsels patient and family on plan of care and related needs.
• Provides clear health instructions per assessment and plan.
• Identifies discharge planning needs and coordinates transition of care.
