Home Sweet Home Health is seeking a compassionate Registered Nurse (RN) to join our mobile Home Health team in Arizona. The RN will perform home health visits, provide skilled nursing, wound care, medication management, and develop individualized care plans for geriatric and adult patients in their homes.

The role offers flexibility, autonomy, and meaningful patient relationships with coverage across East Valley communities including Scottsdale, Mesa, Gilbert, Chandler, and Tempe.

Qualificações

• Current Arizona RN license in good standing.
• Graduate of an accredited nursing program (BSN preferred).
• Minimum of 1 year RN experience required.
• Home Health nursing experience strongly preferred.
• Geriatric patient experience preferred.

Responsabilidades

• Conduct comprehensive nursing assessments in patients’ homes.
• Deliver skilled nursing care including wound care, medication management, treatments, and monitoring.
• Develop, implement, and update individualized home health care plans.
• Educate patients and caregivers on disease management, medications, safety, and ongoing care needs.
• Collaborate with physicians, case managers, therapists, and interdisciplinary care teams.
• Complete accurate and timely clinical documentation in compliance with agency standards.
• Monitor patient progress and communicate changes to the care team.
• Support improved patient outcomes, independence, and quality of life.

Home Health RN Responsibilities

Qualifications

To be considered for this RN Home Health position in Arizona, candidates should meet the following requirements:

• Current Arizona Registered Nurse (RN) license in good standing
• Graduate of an accredited nursing program (BSN preferred)
• Minimum of 1 year of RN experience required
• Home Health nursing experience strongly preferred
• Experience working with geriatric patient populations preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and organizational abilities
• Valid driver’s license, reliable transportation, and ability to travel throughout the East Valley areas
• Ability to work independently while delivering compassionate mobile care
