Angels Care Home Health is seeking an Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will interact with other medical professionals to deliver high-quality care and will perform the initial admission visit in the patient’s home to determine eligibility for home health services.

The role requires an active RN license, current CPR, a valid driver’s license, and reliable transportation.

Formación

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse; two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsabilidades

• Coordinate all aspects of the patient’s home care as RN Case Manager.
• Interact with other medical disciplines as needed to provide professional care.
• Perform admission visit to the patient in the home and determine eligibility for home care.
• Develop and follow the plan of care for each patient.
