Job summary

Adara Home Health, Inc. seeks an RN Case Manager to deliver skilled nursing care and oversee home health services for clients in the community.

You will work with clients, families, and the health care team to develop and implement goal-centered care plans while considering quality measures and financial aspects of treatment. The role requires Minnesota RN licensure, strong critical thinking, documentation accuracy, and the ability to travel to client homes to provide 1:1 care.

Qualifications

• Two+ years of experience in a health care facility or home care setting.
• Demonstrates critical thinking and the ability to follow standards to provide safe client care.
• Ability to work independently with minimal supervision and manage multiple assignments.
• Ability to build and maintain professional relationships.
• Excellent written and verbal communication skills; interpersonal teaching abilities.
• Basic computer skills; experience with EMR and Outlook preferred.
• Current CPR certification is highly recommended.

Responsibilities

• Provide direct nursing care and clinical oversight to licensed and unlicensed personnel.
• Collaborate with clients, families, and the health care team to implement goal-centered care plans.
• Ensure documentation standards are met and compliant with payer and billing requirements.
• Coordinate client care with family, caregivers, and community resources.
