Redcrest Home Health is seeking a Registered Nurse Case Manager (RNCM) in Indianapolis, IN. The role leads patient assessment, planning, and coordination of care across disciplines, with visits to patients at home and ongoing documentation to ensure quality and compliant care.

The RNCM educates patients and families, conducts OASIS assessments, and ensures timely start and continuation of care. A valid RN license and driving are required, with on-call responsibilities as scheduled.

Competenze

• Minimum of one year of RN experience, preferably in home health.
• Ability to communicate effectively with patients, families, physicians and co-workers.
• Strong documentation and organizational skills.
• Valid driver’s license and auto insurance.

Mansioni

• Oversees the overall care of the patient from start to end of episode.
• Updates all doctor orders in patient chart accordingly.
• Notifies doctor of any significant changes in patient’s condition.
• Conducts multi-disciplinary coordination and supervises care providers.
• Provides skilled nursing care in the patient’s home as ordered by the physician.
• Performs OASIS assessments to develop an individualized plan of care.
• Documents all aspects of treatment, assessments, and patient education.
• Ensures compliance with Medicare/Federal guidelines.
• Participates in on-call requirements and case conferences.
• Drives to patient locations and completes visits within required timelines.
