UnitedHealth Group is seeking an RN Case Manager to guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care under supervision of an RN or MD.

You will educate, coach, and provide treatment-decision support; collaborate with a Care Advocate and Social Worker; and plan staff education while performing on-call duties as assigned.

Qualifications

• Current RN license for the assigned state(s).
• Valid driver's license, reliable transportation and liability insurance.
• Current CPR certification.
• 1+ year of clinical RN experience.

Responsabilités

• Assess patient needs and collaborate on care plans.
• Coordinate care, discharge, and transition planning.
• Educate, coach, and support treatment decisions.
• Supervise LPNs and HHAs.
• Complete timely and accurate documentation.
• Communicate patient progress to clinical team.
• Provide staff education and participate in on-call duties.
• Engage in ongoing clinical development.
