Job summary

UnitedHealth Group in Washington, DC is hiring a Nurse Case Manager to guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care under the supervision of an RN or MD.

You will help ensure patients receive appropriate care; provide education, coaching, and treatment-decision support; and collaborate with a Care Advocate and Social Worker to optimize outcomes.

Qualifications

• Current RN license for the state(s) assigned to work.
• Current CPR certification.
• 1+ years of clinical experience as an RN.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitation measures; assists patients in self-care.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and services to patients/families as assigned.
• Participates in clinical development and continuing education.
