Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum. Under supervision of an RN or MD, you will assess needs, coordinate care, support discharge planning, and manage outpatient care while educating and coaching patients and collaborating with a Care Advocate and Social Worker.

Responsibilities include patient assessments, referrals, staff supervision, documentation, and on-call service.

Qualifications

• Current RN license in the state(s) you are assigned to work.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• 1+ years of clinical experience as an RN.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and self-care
• Supervises LPNs and HHAs
• Completes documentation timely and accurately
• Communicates patient progress to the clinical manager and care team
• Plans and provides staff education
• Performs on-call responsibilities and services
• Participates in clinical development and continuing education
• Other duties as assigned
