Job summary

Elara Caring is seeking an RN Case Manager to provide home health care across Youngstown, Warren, Cortland, Poland, Boardman, and surrounding communities in Trumbull and Mahoning counties. You will build relationships with patients, create individualized care plans, and coordinate with physicians to deliver care in patients’ homes.

Requirements include an active Ohio RN license, 1+ year clinical experience, and the ability to travel up to 50% with a valid driver’s license.

Qualifications

• Current, unrestricted RN license in Ohio.
• 1+ year clinical care experience as a nurse.
• Willingness to travel within branch coverage area (50% travel).
• Ability to frequently lift, push, pull, and move up to 50 pounds.
• Valid driver’s license or reliable transportation.

Responsibilities

• Assess patients and develop individualized care plans.
• Provide skilled nursing care in patients' homes.
• Educate patients and caregivers about medications and health management.
• Coordinate care with physicians and the interdisciplinary team.
• Complete timely and accurate clinical documentation.

Elara Caring is hiring an RN Case Manager to provide home health care throughout Trumbull and Mahoning counties.

What You’ll Do

• Assess patients and develop individualized care plans.

What You’ll Need

• Current, unrestricted state RN licensee
• 1+ year experience in a clinical care setting as a nurse
• Able and willing to travel within branch/office coverage area.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Must be able and willing to travel 50%
• Associates Degree or Bachelor’s Degree in Nursing is preferred
• Previous Home Health or Hospice Experience is preferred
• Previous experience with HomeCare HomeBase is preferred
