Job summary

Elara Caring is seeking a Registered Nurse (RN) for home health in Southern Cuyahoga County and surrounding areas in Ohio. You will assess patients in their homes, develop care plans with physicians, and educate families to manage care confidently. Travel between patient homes is expected as part of the role.

The ideal candidate holds an Ohio RN license with a nursing degree and at least one year of clinical experience; home health experience is a plus but not required.

• Current RN license valid in Ohio.
• Graduation from an accredited nursing program (certificate, diploma, associate, or bachelor).
• At least 1 year of nursing experience in a clinical care setting.
• Home health experience preferred, but not required.

Responsibilities

• Complete initial and ongoing assessments to identify patient needs.
• Provide nursing care according to individualized care plans and physician orders.
• Monitor patient progress, report changes, and take appropriate action.
• Teach patients and families how to manage care at home.
• Communicate with physicians and care team to support coordinated, continuous care.
• Maintain accurate clinical documentation and protect patient and staff privacy.
• Support safe, quality care through timely follow-up and clinical judgment.

Help patients recover, understand their care, and feel more confident at home. Elara Caring is seeking a Registered Nurse to provide home health care throughout Southern Cuyahoga County.

You’ll build relationships with patients and families, use your clinical judgment, and coordinate care around each patient’s needs. Home health experience is preferred, but not required, making this an opportunity to bring your nursing skills to a new setting.

What You’ll Need

• Current RN license valid in Ohio.
• Graduation from an accredited certificate, diploma, associate, or bachelor’s nursing program.
• At least 1 year of nursing experience in a clinical care setting.
• Home health experience preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
