Tidewell Hospice, part of Empath Health, is hiring an RN Case Manager for the Facilities Team in Bradenton, FL. You’ll work under a licensed physician to relieve symptoms and manage care for patients with advanced illness, coordinating with the interdisciplinary team to implement individualized plans.

Requires Florida RN license, CPR, and at least one year in acute care; CHPN encouraged. Monday–Friday 8a–5p with coverage in Manatee and North Sarasota counties.

Qualifications

• Graduate of approved nursing program and current unrestricted Florida RN license.
• CPR certification required.
• BSN or MSN preferred; experience in hospice is advantageous.

Responsibilities

• Promotes and practices the Hospice mission and core values.
• Under physician supervision, manages discomfort and provides symptom relief in palliative care.
• Assesses impact of terminal diagnosis on physical, functional, psychosocial needs and ADLs.
• Collaborates with IDG to develop and update the plan of care and initiate referrals.
• Educates patient, family, and caregivers on disease process and care.
• Adheres to facility customer service acknowledgement processes.
