Job summary

Empath Health's Tidewell Hospice is seeking an RN PRN to join the team.

The PRN RN Case Manager will coordinate care for hospice patients and families, visiting in the home, long-term care facilities, or assisted living facilities in South Sarasota and Charlotte counties of southwest Florida.

Qualifications

• Graduate of an approved nursing program.
• Florida RN license required; BSN/MSN preferred.

Responsibilities

• Create and maintain a plan of care as the care coordinator for hospice patients and families.
• Review and update care plans, physician orders, and records; educate patients and caregivers.
• Provide nursing care in home, long-term care, or assisted-living settings for hospice patients.

Empath Health is currently seeking an RN PRN to join our team.

Coverage area includes the South Sarasota and Charlotte county.

What You’ll Do

• The PRN RN Case Manager creates and maintains a plan of care, serving as the coordinator for patient and family interaction with Tidewell for the duration of the patient’s hospice experience. When a patient and loved ones think of Tidewell Hospice, they will think first of the RN Case Manager. The case manager must be able to listen, observe, communicate and sensitively provide nursing services in emotionally intense situations.


• The PRN RN Case Manager reviews and updates the care plan, physician orders, pharmacy profile and other records. The nurse also educates patients, family members and other caregivers about the disease process and decline, available palliative interventions, the dying process and safety practices.


• The successful applicant will serve hospice patients and their families in the home, long-term care facility, or assisted-living facility.


• Applicants must be a graduate of an approved nursing program and own a current Florida RN license. BSN or MSN preferred. Minimum one year experience in an acute-care hospital environment providing direct patient care; previous experience as a hospice RN preferred.

What You’ll Need

• Bedside hospital, ICU, ER or other inpatient experience within the past 10 years.


• Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; Minimum of or six months to one year related experience and/or training; or equivalent combination of education and experience.


• Licensure:


• Valid Florida license as a Registered Nurse


• Valid Florida State Driver’s License


• Minimum of 1 year of current active nursing experience; with experience in caring for terminally ill patients or those with life threatening illnesses preferred; prefer one (1) year experience in an acute or post acute care setting.
