Job summary

Henry Ford Health in Michigan seeks a Case Manager for an on-site, full-time day shift. You will coordinate care, develop comprehensive care plans, and support safe transitions across the care continuum.

Ideal candidates are RNs or LMSWs with case management experience; CCM or ACM certification preferred; EPIC/Care Port experience a plus. Orientation is 6–10 weeks, 7:30am–6pm, with every 4th weekend and holiday rotation.

Qualifications

• Minimum 3 years of clinical experience.
• Excellent customer service and interpersonal skills, including the ability to interact with internal and external customers at all levels.
• Strong problem-solving, analytical, and decision-making skills.
• Strong computer skills and knowledge. EPIC & Care Port experience preferred.
• Experience in discharge planning, home health care, rehabilitative medicine, or managed care preferred.
• Knowledge of preventive service guidelines, clinical practice guidelines, behavior change theory, Medicare and Medicaid regulations and case management principles.
• Knowledge of medical ethics and legal implications related to case management.
• Understanding of social determinants of health and their impact on a patient’s health.
• Well versed in facilitating community resources to meet the needs of diverse populations.
• Strong organizational, planning and implementation skills with the ability to handle multiple complex patients’ needs simultaneously.
• Strong sense of compassion with the ability to successfully advocate for patients and their families.

Responsibilities

• Create comprehensive care plans that integrate patient and family preferences and values.
• Monitor patient medical necessity and level of care through assessments and ongoing evaluations.
• Advocate for resources and remove barriers to ensure effective care delivery.
• Maintain ongoing dialogue with supervisors and care transition team members to reevaluate and implement health plans.
• Serve as a resource for medical record documentation, level of care recommendations, and post-discharge treatment options.
