Job summary

Henry Ford Health Genesys Hospital is seeking a Case Manager to join the care team. The role focuses on patient-centered coordination, discharge planning and advocacy to ensure safe transitions and quality, cost-effective outcomes across the care continuum.

The position requires an RN with a Michigan license or a Master’s in Social Work, plus 3+ years of clinical experience and strong communication, documentation and analytical skills.

Qualifications

• Bachelor’s degree in nursing or a Master’s degree of Social Work.
• Minimum 3 years of clinical experience.
• Excellent verbal communication and written documentation skills.
• Excellent customer service and interpersonal skills.
• Experience in discharge planning, home health care, rehabilitative medicine, or managed care preferred.
• Knowledge of Medicare and Medicaid regulations and case management principles.
• Understanding of social determinants of health.
• Ability to facilitate community resources for diverse populations.
• Strong organizational and problem-solving skills.

Responsibilities

• Creates plan for care across the continuum, integrating patient/family preferences and values.
• Monitors patient medical necessity and level of care through assessments, ongoing evaluations and/or patient records.
• Advocates for resources and removal of barriers.
• Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
• Acts as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.
