Job summary

Henry Ford Health is seeking a Case Manager to join our patient-centered care team. You will coordinate care across transitions, identify needs, plan and facilitate services, evaluate outcomes, and advocate for patients and families to ensure safe, cost-effective care.

The ideal candidate holds an RN or LMSW with Michigan licensure, CCM/ACM preferred, and at least three years of clinical experience, plus strong communication and computer skills.

Qualifications

• Bachelor's degree in Nursing or Master's degree in Social Work.
• Minimum three (3) years of clinical experience.
• Excellent customer service and interpersonal skills with ability to interact with internal and external customers at all organizational levels.
• Strong problem-solving, analytical, and decision-making skills.
• Strong computer skills and technical knowledge.

Responsibilities

• Identify, assess, plan, facilitate, and advocate for patient care options and services.
• Coordinate care across hospital or specialty-based care teams to meet needs.
• Provide effective discharge planning to support safe transitions in care.
• Support cost-effective outcomes and promote patient safety and quality of care.
