Job summary

Henry Ford Hospital - Detroit Main Campus is seeking a Case Manager-RN to coordinate comprehensive patient care and ensure smooth transitions across care levels. You will work to optimize outcomes through strong clinical judgment, teamwork, and effective communication.

The role includes six weeks of orientation with a weekend component, on-site shifts, and weekends opportunities. A BSN is preferred; ADN with case management experience is acceptable, with CCM/ACM certification within a year.

Qualifications

• BSN preferred or ADN with 2+ years acute care case management experience.
• ADN candidates must enroll in an accredited BSN program within 3 years of hire and complete it within 5 years.
• minimum of three years of recent acute care nursing experience.
• Certification in Case Management (CCM) or ACM within 1 year of hire.

Responsibilities

• Conduct comprehensive patient assessments to identify needs and develop care plans.
• Collaborate with healthcare team for timely interventions and referrals.
• Monitor progress, address barriers, and educate patients and families.
• Develop discharge plans addressing physical, social, and financial needs.
• Coordinate with community resources for post-acute care transitions.
• Advocate for patient rights and access to services.
• Analyze outcomes and contribute to quality improvement.
