Ideal candidates hold a BSN (or ADN with BSN plan) and CCM/ACM certification within one year; acute care experience is preferred. This is a part-time, benefit-eligible position with weekday daytime hours.

Qualifications

• BSN preferred for Case Manager-RN role.
• ADN acceptable if enrolled in BSN within 3 years and complete within 5 years.
• CCM/ACM certification within 1 year of hire is required.

Responsibilities

• Coordinate comprehensive clinical care and assess patient needs.
• Develop discharge plans addressing physical, social, and financial needs.
• Collaborate with interdisciplinary teams and community resources.
• Promote quality improvement and ensure regulatory compliance.
