Qualifications
- Graduate of an accredited school of nursing.
- Minimum three years of clinical experience; two years in Utilization Review/Utilization Management or Case Management preferred.
- Knowledge of social and physical factors affecting discharge planning and knowledge of community resources.
- Ability to provide age-appropriate care and documentation.
Responsibilities
- Achieve quality healthcare outcomes by safe, individualized discharge planning.
- Provide proficient timely utilization management to maximize reimbursements.
- Collaborate with patient, family, and healthcare team to meet post-discharge needs.
