Qualifications

• Graduate of an accredited school of nursing.
• Minimum three years of clinical experience; two years in Utilization Review/Utilization Management or Case Management preferred.
• Knowledge of social and physical factors affecting discharge and community resources.
• Proficiency with word processing, spreadsheet, and data collection/management software.

Responsibilities

• Establish safe, individualized discharge planning.
• Provide proficient timely utilization management services to maximize reimbursement for patient visits.
• Collaborate with patient, family and healthcare team to achieve quality healthcare outcomes.
