Job summary

Novant Health's Neuro Unit in Winston-Salem, NC is seeking a dedicated RN Case Manager for a full-time day-shift role. You will coordinate complex discharge planning, work closely with a multidisciplinary team, and advocate for patients and families navigating transitions of care to the next level of care.

The ideal candidate holds an Associate Degree in Nursing (BSN preferred) and an active RN license in North Carolina, with 2+ years of healthcare or case management experience preferred.

Qualifications

• Two or more years healthcare experience, preferred.
• Case Management or related experience, preferred.
• Current RN licensure in state of employment, required.

Responsibilities

• Lead complex discharge planning and care coordination.
• Collaborate with physicians, nurses, MSWs and other healthcare professionals on the Neuro care team.
• Advocate for patients and families to ensure safe transitions to the next level of care.
• Support medically complex neurological patients and their unique needs.
• Engage in problem-solving and collaborative care planning to improve patient outcomes.
