Job summary

Northeast Georgia Health System in Georgia is seeking a Registered Nurse with experience in care coordination to lead assessment, planning, and evaluation of patient needs. The role involves collaborating with physicians, nursing staff, utilization review, and the care team to align care with quality and cost objectives, and to manage length of stay and transitions across the continuum.

Qualifications

• Licensed to practice as an RN in Georgia.
• Associate degree from an accredited nursing program.
• 3–5 years of direct patient care and/or case management experience.
• Financial and discharge planning experience.
• Adherence to patient safety guidelines and regulatory requirements.

Responsibilities

• Monitor patients on assigned units to manage resources and length of stay.
• Communicate LOS and patient progress with physicians, patients and families.
• Coordinate and facilitate discharge plans and post-acute needs.
• Collaborate with Utilization Review and multidisciplinary teams to ensure appropriate care.
• Lead discharge planning, social services, and community referrals on the unit.
• Maintain sense of urgency and provide cross-coverage including weekends.
