Job summary

Northeast Georgia Health System is seeking an experienced RN to provide comprehensive assessment, planning, and coordination of patient care across units. The role includes discharge planning, collaboration with physicians and families, and participation in interdisciplinary rounds to ensure timely, efficient, and high-quality care across ages from neonate to geriatric.

Qualifications

• Licensed to practice as an RN in Georgia.
• Associates Degree in Nursing; graduate of an accredited nursing program.
• 3–5 years of direct patient care and/or case management experience.
• CCM/ACM certification preferred.

Responsibilities

• Monitor patients on assigned units and manage length of stay.
• Coordinate discharge planning and post-hospitalization needs.
• Communicate with physicians, patients, families and care team regarding progress and discharge.
• Ensure regulatory compliance and participate in interdisciplinary rounds.
• Maintain knowledge of reimbursement methodologies and payer guidelines.
• Support a customer service oriented environment and effective hand-offs.
