Job summary

Northeast Georgia Health System is seeking a skilled Registered Nurse to provide comprehensive patient assessments, planning, and care coordination. You will work with physicians, families, and the care team to ensure efficient, high-quality outcomes while managing length of stay and discharge planning across age groups.

The role requires GA RN licensure, an accredited nursing program background, and 3–5 years of direct patient care or case management experience.

Qualifications

• Licensed to practice as an RN in Georgia.
• Associate degree; graduate of an accredited school of nursing.
• 3–5 years of direct patient care and/or case management experience; financial and discharge planning experience.
• CCM (Case Management Certification) or ACM (American Case Management Certification) preferred.

Responsibilities

• Monitor patients on assigned units to ensure appropriate use of resources and interventions while managing length of stay.
• Communicate with physicians, patients/families, and other disciplines about discharge timelines and progress.
• Coordinate and facilitate discharge planning for post-hospitalization needs.
• Maintain knowledge of reimbursement methodologies and third party payer guidelines.

Education

Associate degree in Nursing

Graduate of an accredited school of nursing

Bachelor's degree (preferred)
